From this link, complete the ADMIN QUERY section. Please ensure you put in as much detail as possible otherwise your request may be delayed. Once you have submitted your request through the link above, you can expect a response from us within 5 working days via text, phone call or you may be advised to make a routine appointment.  If the online access is closed, please submit your request the next working day.

Any requests sent by patients via the email will not be actioned.
